Rules --

01. You must be a fan of the Grey's Anatomy Interns (duh...)
02. You don't have to have a site, but if you do, you must place one of my codes on your site before or right after submitting your form. If I don't see a code when I visit your site, I will still list you but I will not include your website. If you own an inappropriate site (i.e. hateful, porn, illegal, e.t.c.) just say you don't have one, because I won't list it anyways.
03. Provide me with accurate information please.
04. Notify me of changes with the edit form, not the join form.
05. Do NOT direct link the codes! Save them to your computer and upload them to your own server!!
06. Fill out all required fields of the form.
